SO far as what I have heard, Buffy Anne Summers is really Buffy's name. However the are a few fics out there that put forth the idea that Buffy is short for Elizabeth. Although I'm not sure how that came about.
Faith has never had a last name on the show. A couple of ones I have heard in Fic are Faith Spencer (not sure where it came from) and Faith Wilkins (She took the last name of the Mayor as if he adopted her).
